Eye Movements in Unconscious Patients

Abstract

Unconscious patients such as coma are unresponsive to all external stimuli, noxious or otherwise. Neuro-ophthalmologic examinations are paramount in the neurologic assessment of those patients, especially with regard to brain stem localization and diagnosis. Examination of the pupils, fundi, and corneal reflexes are all helpful. However, the ocular motility examination is especially important because the pathways governing ocular motility traverse the entire brainstem, so pathology in this region often produces recognizable eye movement abnormalities. The evaluation of eye movements consists of next three steps. Observing the resting position of the eyes, evaluating spontaneous movements, and investigating reflex eye movements. This article details the eye movement examination in unconscious patients.
